Issue of Certificate

Issuing Certificates (Current Students)

Certificates are available through the automated certificate-issuing machines at each campus.
Operating hours at Nakano Campus are

Weekday 9:00 ~ 19:30
Saturday 9:00 ~ 14:00
※If a certificate has to be enclosed in an official envelope, please bring the certificate to counter no.1 at the Nakano Campus Office.

Issuing Certificates (Graduates)

There are two ways to make an application.
1. In person: Visit counter no.1 at the Nakano Campus Office.
(Nakano Campus: 3rd floor, Low Rise Wing)
2. Apply by mail.
※ Applications cannot be made by telephone, email or fax.
※ If another person is making the application on behalf, an authorization letter from the applicant is required. Please refer to Applications on behalf for details about the authorization letter.
※ The graduate's name which was registered while they attended the university will be printed on the certificate. 
※If the family name has changed after graduation and there is a need to issue a certificate with the new family name, please bring or send an official copy of the Family Register.

★Application in person at Nakano Campus Office:

Please submit the following items to counter no.1 in the Nakano Campus Office.
1) Application form for Certificates (A4 size. Written in Japanese only. Please refer to the English sample.)
2) Issue fee (see chart below)
3) ID with photo (passport, residence card or driver's license)
※ If the certificate needs to be provided in an officially sealed envelope, please indicate this on the application form.
※ Certificates are processed within 10 to 20 minutes.

★Application on behalf

If another person is making the application on behalf, the following documents are also required, in addition to the items above.
1. Authorization Letter from the applicant
2. Photocopy of the applicant's ID with photo
3. Photocopy of ID with photo of the person making the application on behalf of applicant

In the Authorization Letter, please make sure to include the following items a) - d):
a) The name, home address, contact information (telephone number, etc.) of the person who is applying on behalf of the applicant.
b) Specify the items to be accessible by the person applying on behalf. (All authority for applying and receiving the certificate)
c) The date the authorization letter was created.
d) The applicant's name (handwritten by the applicant), seal, current address, date of birth, graduation date, department enrolled, and contact information (telephone number, etc.)

There is no required format, but you can use this form.

★Application by mail

 Please send the 4 following items to Nakano Campus Office.

1) Application for Certificates (A4 size. Written in Japanese only. Please refer to the English sample.)
2) Issue fee (see chart below): By domestic money order for the exact amount, or domestic registered mail for cash.
3) Copy of ID with photo (passport, residence card or driver's license)
4) A self-addressed envelope with stamps (The stamps should be attached, and the address of the recipient should be written.)

※ If the certificate needs to be enclosed in an officially sealed envelope, please indicate this on the application form.
※ The certificate will be sent within 3 or 4 days after the application has been received at the Nakano Campus Office.

※Issuing commission by mail: Either a postal money order for the exact amount or registered mail for cash.
