Certificate issued for Current student

Automated certificate-issuing machines are available at each campus.

Weekday 9:00 ~ 19:30
Saturday 9:00 ~ 15:30
※If a certificate has to be enclosed in an official envelope, please bring the certificate to the Nakano Campus Office.

Certificate issued for Graduates

Two ways to make an application
1. In person: Visit Nakano Campus Office (Nakano Campus: 3rd floor, Low Wing Building).
2. Apply by a mail.
※Applications cannot be made by fax, e-mail or telephone.
※If an application is made through an attorney, a Proxy statement that has the applicant’s seal on it must be presented together with the necessary application documents.
※The certificate should have the applicant’s name while they attended the university. If the graduate’s family name changed after graduation and the applicant needs a certificate with the new family name, he or she must bring or send an official copy of Family Register.

★Application in person at Nakano Campus Office:

Please fill in the prescribed form and submit it to the office.
The following are needed.
①Application for Certificates (A4 Size. Written in Japanese only. Please refer to the English sample.)
②Application fee (Refer to the following table.)
③One means of photo identification (Driver’s license, passport, etc.)

※Please note that the identification should NOT include any social security number (Ex. MY NUMBER)                 
※If there is no photo identification, please bring two other means of identification (Ex. residency card, health insurance card, etc.). 
※If the certificate has to be hermetically sealed, please write this down on the application form.
※Please click here for the opening hours of the automated certificate-issuing machines (Open weekdays until 17:00 through to March 31st).
※Process generally takes 10 to 20 minutes.

 

★Application by mail

Please send the following 4 documents to Nakano Campus Office.
Application for Certificates (A4 size. Written in Japanese only. Please refer to the English Sample)
②Application fee (Refer to the following table.)
③One photo identification (Ex. copy of driver’s license, passport, etc.)
④Stamped, self-addressed envelope (This should have an attached stamp as well as the name and address of the recipient.)

※Please note that the identification should NOT include any social security number (Ex. MY NUMBER) 
※If there is no photo identification, please bring two other means of identification (Ex. residency card [Jumin-hyo], health insurance card, etc.)
※If the certificate has to be enclosed in an envelope, please write this down on the application form. 
※Submission of fees by mail: Either a postal money order for the exact amount or registered mail for cash.
※The document will be returned within 3 to 4 days after the application has been received by Nakano Campus Office.

★Proxy statement and Identification (when an attorney makes the application)

In the proxy statement, please make sure to include the following a) to d):
※When completing the application form, the attorney must bring his or her ID.
a) The attorney’s name, home address, contact information (telephone number, etc.)
b) Designate the items for which the attorney has the authority to act.
c) Date of proxy statement
d) The applicant’s name (hand written), seal, current address, date of birth, graduation date, department in which enrolled, and contact information (telephone number, etc.)
