Application by a person (proxy) other than the applicant
(1) Letter of proxy (free format)
* Items to be noted in the letter of proxy
A. Information on the proxy
Address, name, date of birth, contact information (telephone number, etc.)
B. Items of delegation
Statement to the effect that the delegator is delegating all authority in issuance of application for and receipt of the certificate(s) to the proxy
C. Date of preparation of the letter of proxy
D. Information on the delegator
Address, name (written by the delegator him/herself), seal impression, date of birth, date of graduation, school of registration, etc., contact information (telephone number, etc.)
(2) Confirmation of proxy identity
An identification card with a photo of the proxy’s face
* Presentation of at least two other identification documents if the proxy does not have an identification card with a photo of his/her face
(3) Confirmation of delegator identity
Copy of an identification card with a photo of the delegator’s face
* Presentation of copies of at least two other identification documents if the delegator does not have an identification card with a photo of his/her face
