Please make sure both the applicant's signature and the coordinator’s signature are included on the document. The form can be found in the folder. |

Copy of Passport (PDF/JPG) |
The passport must be valid at the time of application. If the applicant's passport is close to expiring, the Immigration Services Agency of Japan may require the passport to be renewed, which could delay the issuance of the Certificate of Eligibility (CoE). |

Academic Transcript -GPA 2.3 or higher is required. Please refer to the ※1 Conversion Table below. |
Must be written in English or Japanese. The applicant's GPA must be 2.3 or higher based on Meiji University’s grading system. The latest official transcript showing all grades earned at the applicant's home university must be submitted. |

Should be completed by a doctor within 1 year prior to enrollment. |

University Dormitory Application Form |
To apply for university housing, carefully review the “Dormitories” section of the Fact Sheet before completing the University Dormitory Application Form. All dormitory fees are non-refundable under any circumstances after payment has been made. Dormitory fees at Meiji University are generally charged on a per-semester basis. The fee for the first semester (approximately five months) must be paid in full before arrival. For students staying for one year, the fee for the second semester must be paid in full after arrival. |

Digital Color Portrait Photo (JPEG) (JPEG/JPG format only; no PDF or HEIC files; file size 1-3 MB) |
1) The photograph must be submitted in JPEG or PDF format with high resolution (file size between 1MB and 3MB). 2) The photograph must be a recent photo of the applicant, taken within the past 6 months, with a white or light-colored background. It must meet standard ID photo requirements. |

Certificate of English Proficiency |
The applicant is required to submit the Certificate of English Proficiency issued by the applicant's university in place of an English test score if the applicant meet all of the following conditions: (1) The applicant enrolled in a degree program that is taught entirely in English. (2) The applicant's university is located in a country where English is not the primary native language. (3) The applicant does not have a valid English test score (e.g., TOEFL iBT or IELTS). |
